In-Person Medical Card Certification with nuEra Urbana

Get certified for the Illinois Medical Card Program in-person with the help of nuEra Urbana. This event is walk-in, first come first serve.

MED certification cost is $75 paid during the event. (NO CASH! Debit, Credit, Venmo, PayPal etc accepted)

What to bring:

1. Valid and accessible email address 
2.  Valid Driver’s License/State ID (If your address does not match the one listed on your ID, please bring another proof of residence) 
3. If you are a Veteran, please bring a copy of your DD214 
4. If you are receiving care at a VA facility, please bring a copy of the last 12 months of your medical records from the VA (available online) 
5. If you receive SSI/SSDI, please bring a copy of your Benefit Verification Letter dated within the last 12 months 
6.  If you are 65+ years of age, you DO NOT need to submit any additional documentation to receive a discounted application fee 
7. Please bring a debit or credit card for payment of State fees (cash not accepted) 

After your certification nuEra staff will help finish the application and you could have your medical card that same day!
